What Is DOMS and Why Does It Happen?
Delayed onset muscle soreness (DOMS) is the muscular pain and stiffness that peaks 24–72 hours after unaccustomed or high-intensity exercise. It results from microscopic muscle damage during the eccentric (lengthening) phase of muscle contractions — the phase that triggers adaptation.
DOMS is a normal consequence of productive training. Muscles that are sufficiently challenged to produce adaptation are also muscles that experience temporary damage and subsequent inflammation. The soreness is a byproduct, not the goal.
Understanding this prevents two common mistakes: avoiding training altogether to prevent any soreness, or treating severe soreness as always acceptable and training through what may be genuine injury.
Productive Soreness vs. Warning Signs
Productive DOMS characteristics:
- Diffuse, bilateral (affects both limbs equally)
- Peaks 24–72 hours post-exercise, then resolves
- Stiffness that loosens with movement
- Located in the primary muscles worked
Warning signs requiring evaluation:
- Sharp, localized pain (not diffuse)
- Pain that gets worse with movement (not better)
- Asymmetrical — one limb significantly more painful than the other
- Pain in joints, not muscles
- Doesn't improve within 5–7 days
When in doubt, refer to medical staff.
Evidence-Based Soreness Management Strategies
Active recovery: Light movement (walking, easy cycling, swimming) increases blood flow and accelerates DOMS resolution. The temporary discomfort of movement is worth it.
Cold water immersion: 10–15°C water for 10–15 minutes reduces acute soreness and perceived pain. Most effective within 2–4 hours post-exercise for maximum sessions.
Contrast therapy: Alternating cold (1 min) and warm (3 min) water, 4–6 cycles. Comparable effectiveness to cold immersion with better tolerance.
Compression: Compression garments worn for 12–24 hours post-exercise modestly reduce DOMS and swelling.
Massage and foam rolling: Self-myofascial release (foam rolling) has modest evidence for reducing perceived soreness. Effect size is small but consistent, and technique tolerance is good.
Nutrition: Adequate protein (see Recovery Nutrition article) supports repair. Tart cherry juice has evidence for reducing DOMS in short-duration high-intensity exercise.
Sleep: The most important recovery factor. Growth hormone release during deep sleep is when most repair occurs.
Training Through Soreness
Should athletes train when sore? It depends on severity and timing:
Mild-moderate DOMS: Train. Reduce load if necessary. Active work reduces soreness faster than complete rest.
Severe DOMS (significantly limits range of motion): Reduce intensity and volume significantly. Active recovery focus. Allow another 24–48 hours before full training.
Progressive overload and soreness: As athletes adapt to a training stimulus, soreness from that stimulus decreases. This is expected — it doesn't mean training stopped working. DOMS is greatest at the start of a new phase, then diminishes.
Team-Wide Soreness Management
For coaches managing a team through a heavy training block:
- Proactively communicate that soreness is expected and normal
- Monitor athletes who are significantly more sore than teammates (potential injury risk or poor recovery habits)
- Build active recovery sessions into the weekly plan rather than adding them reactively
- Schedule heavy training blocks when significant competition is at least 5–7 days away
